Why the heck am I doing this?  Well, frankly, people are constantly changing addresses, phone, emails, jobs, locations, situations at such a constant rate and my address book is out of date as soon as I jot the stuff down.  So I figured I'd make it easier for people to keep me in the loop - hence this form.

Could you take a minute and fill out your contact information?   It'll get sent to me as an email (and you'll get a copy), and I use that to update my household address book.   Fill out as much or as little as you feel comfortable (please include area code in phone numbers, and country code outside the US).  Obviously, I will treat this as private and not sell or give out this info to others.
